We are seeking a compassionate and dynamic individual to join our team as a Care Coordinator for the PIVOT program. In this role, you will have the opportunity to coordinate care for our patients, making a meaningful difference in their lives and supporting them on their journey to recovery. • RESPONSIBILITIES: include the following. Other duties may be assigned. Responsible for the development of care planning and patient needs assessments. Facilitates life skills groups in accordance with the CTP policies and procedures. Acts as liaison, establishing networks and collaboration with hospitals, community health centers, the state, and human service agencies about patient services and needs. Document admissions, care management needs, discharge, and discharge planning. Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake. Organize a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators. Assists treatment team with patient intake, transfer, and discharge assessments. Collaborating with the facility treatment team (medical, psychiatric, and SUD) to identify patients’ needs. Assist patients in meeting their basic needs, such as clothing, medical care, legal support, and religious services.
• QualificationsQUALIFICATIONS:
• Education: A degree in a behavioral health-related field, such as an associate or bachelor’s degree, is preferred. Obtain the Certificate of Addiction Technician (CAT) within 18 months from the hire date. Obtain case management certification within 90 days of hire.

Experience

Minimum of two years of experience working in the substance misuse and mental health field.
