The Case Manager provides community-based support and service linkage to help individuals address behavioral health needs and developmental issues. They collaborate with parents, guardians, and outside providers to track health outcomes and ensure whole-health support.
Candidates must have a bachelor's degree in a related field or three years of human services experience, along with a high school diploma/GED and a valid Ohio driver's license. Experience with autism spectrum disorders, developmental disabilities, and mental health conditions is preferred.
