The Health Coach provides education and behavior change support to heart failure patients within a value-based care model. They collaborate with clinical teams to improve patient self-management and engagement while maintaining non-clinical boundaries.
Candidates must hold an NBC-HWC certification and preferably a bachelor's degree in health sciences. Experience in chronic disease management, cardiology, or telehealth is highly desired.

The Health Coach is a non-clinical member of the Heart Failure care team, supporting patients through education, behavior change, and engagement within Novocardia’s value-based care model. This role partners closely with clinical team members to empower patients to better understand and manage their heart failure through prevention-focused, patient-centered coaching.
The Health Coach plays a key role in improving patient experience, engagement, and self-management while maintaining clear boundaries from clinical decision-making and care delivery.
