• What you'll do
• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing visits and ongoing patient evaluations.
• Educate patients and caregivers on disease management, medications, and self-care.
• Coordinate care with physicians, therapists, and interdisciplinary team members.
• Monitor patient progress and communicate changes in condition.
• Supervise Home Health Aides and LPN/LVNs as appropriate.
• Maintain accurate clinical documentation and ensure compliance with regulatory standards.
• Help patients safely remain in their homes while achieving their healthcare goals.

Qualifications

• Current RN license.
• Current CPR certification.
• Valid driver's license and auto insurance.
• Home Health experience preferred.
• Homecare Homebase experience is a plus.
• Passion for patient-centered care and clinical excellence.
