Must be a graduate of an accredited Occupational Therapy or Occupational Therapy Assistant program with a current or eligible license. Experience in skilled nursing or long-term care is preferred.

Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) – Cherokee, IA Blue Stone Therapy is seeking an Occupational Therapist or an Occupational Therapy Assistant to provide rehabilitation services to adult and geriatric patients at Accura HealthCare of Cherokee in Cherokee, IA. This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The OT or COTA will work as part of an interdisciplinary rehabilitation team to improve activities of daily living, functional independence, upper extremity strength and coordination, cognitive function, and overall quality of life.
] This position will serve multiple partner communities, including [Cherokee Specialty Care] and [Accura HealthCare of Aurelia], with travel between locations as needed.
What You’ll Do Perform occupational therapy evaluations and develop individualized plans of care Provide skilled occupational therapy treatment focused on functional and measurable outcomes Address ADLs, functional mobility, cognition, strength, positioning, and discharge planning Supervise Occupational Therapy Assistants in accordance with regulatory and company standards Maintain appropriate patient schedules per established guidelines Complete timely, accurate documentation to support quality care and reimbursement requirements Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
