Conduct comprehensive physical therapy evaluations and implement individualized treatment plans for patients in their home environments. Collaborate with multidisciplinary healthcare teams to monitor progress and educate families on therapeutic exercises.
Requires a Master's degree in Physical Therapy and a valid state license. Previous experience in home health or outpatient settings is preferred along with proficiency in electronic health records.
Provide comprehensive physical therapy evaluations and ongoing assessments for patients in their home environments. Develop and implement individualized treatment plans based on evaluations and clinical goals. Educate patients and their families about therapeutic exercises, activities, and the maintenance of their physical health. Monitor patient progress and modify treatment plans as necessary to ensure optimal outcomes. Work collaboratively with other healthcare professionals to coordinate care and improve patient outcomes. Document patient care and progress according to regulatory standards and company policies. Stay current with best practices and advancements in physical therapy to incorporate into treatment. Master's degree in Physical Therapy from an accredited program. Valid state licensure as a Physical Therapist. Previous experience in home health care or outpatient clinical setting preferred. Strong interpersonal and communication skills to build rapport with clients and their families. Ability to work independently while managing a varied caseload effectively. Familiarity with electronic health records and documentation standards. Commitment to upholding the company's values of compassion, honesty, and patience.
