Conduct comprehensive in-home assessments and develop person-centered service plans for members of the MyCare Ohio Waiver Program. Coordinate long-term services and monitor compliance with Medicaid regulations to ensure appropriate care in community settings.
Requires an active Ohio LSW or RN license and at least one year of experience in home care, disability services, or case management. Must possess a valid driver's license, reliable transportation, and the ability to obtain Ohio Department of Aging certification.

The Area Office on Aging of Northwestern Ohio (AOoA) is dedicated to helping older adults and individuals with disabilities remain healthy, independent, and living in their homes and communities. Serving over 30,000 individuals annually, our team is committed to delivering compassionate, person-centered care across our region.
Care Manager (also known as a Waiver Service Coordinator) is responsible for conducting in-home assessments, identifying support systems, and coordinating long-term services through the MyCare Ohio Waiver Program. This role ensures members receive appropriate care in home- and community-based settings while maintaining compliance with Medicaid and program regulations.
This position is primarily office-based in a climate-controlled environment with regular in-home visits. Requires extended computer use and occasional lifting to 25 lbs.
Hiring is contingent upon successful completion of a drug screen, background check, and reference check. AOoA is a drug-free workplace. Employment is contingent upon funding availability.
