Range: $24.25 - $25.75 HourlyCase Manager II - Waiver Case Management (Scott County) People Incorporated is seeking a Case Manager II to join our Waiver Case Management- Scott County team to work with clients who are diagnosed with chronic mental illness or other ongoing health conditions. We connect people with services by developing person-centered Support Plans and authorizing services that help them live as independently as possible and integrated within their communities. Waiver Case Management involves more behind-the-scenes service coordination than traditional case management Our Case Managers see clients twice annually and develop long term relationships by developing dynamic service plans. Our staff operate autonomously, creating their own hybrid schedules after completing initial training. We value work-life balance and have manageable caseload sizes that serve 30 individuals on average!
