Oncology nurse navigator-Ppn

• Summary The Oncology Nurse Navigator is a registered nurse (RN) who provides comprehensive, patient‑centered care coordination, navigation, and direct clinical nursing services across the outpatient oncology and ambulatory care continuum.
• This role supports patients and families from diagnosis through treatment and follow‑up, ensuring care is timely, appropriate, safe, and coordinated while optimizing patient outcomes, experience, and quality measures.
• The RN practices to full scope of licensure, serves as a clinical expert and patient advocate, partners with multidisciplinary teams, and supports quality improvement initiatives in alignment with organizational goals and regulatory standards.
• The RN partners with and advocates for patients/families/significant others to provide physical, emotional and spiritual support which impacts patient outcomes.
• The RN is responsible for seeing that patients receive treatment services that are timely, appropriate, and comprehensive.
• This is accomplished through identification of patients’ needs and working collaboratively with a multi-disciplinary team across the in-patient and out-patient continuum of care.
• The RN supports patient/family education, monitors the recommended treatment plan, and sees that appropriate follow up occurs in the outpatient setting.
• The RN also monitors identified quality measures and works with the multi-disciplinary team to reach expected outcomes.
• The RN functions in a versatile role that demonstrates professionalism and promotes excellence and autonomy in the practice of Nursing.
• The RN utilizes the nursing process in conjunction with evidence-based principles to care for a diverse caseload of patients within the department's scope of service.
• The RN serves as guide, healer, collaborator, teacher, leader and sentry.
• The RN collaborates with and guides care delivered by the inter-professional team.
• The RN is accountable for compliance with the Ohio Nurse Practice Act, Nursing Code of Ethics, and applicable regulatory standards.
