The Care Manager facilitates communication of health information and provides clinical documentation and support for patients in Care Management programs. They are responsible for triaging, coordinating care, and developing care plans in collaboration with Primary Care Providers.
Requires 2 to 3 years of experience in primary care, post-acute settings, or patient navigation. Candidates must be a Certified Medical Assistant, Health Coach, or hold an Associate or Bachelor's degree in a related field.
Under the direction of Value Based Care Leadership, the Care Manager 1 provides and facilitates communication of health information and performs clerical and clinical documentation and other support services for patients in Care Management programs. The incumbent is responsible for triaging, coordination of care, documentation, communication, and tracking patient calls, cases and records. They engage patients and their families and/or representatives for disease management and education sessions to promote positive health and behavioral modifications, assists in the development of care plans, and encourages and facilitates engagement with the Primary Care Provider, specialists, and other community-based healthcare providers. The Care Manager 1 provides information for basic social services, application assistance, and care planning for patients, as needed. He/she provides transition of care services to patients being discharged from post-acute settings, such as hospitals and skilled nursing facilities. He/she is responsible for ensuring billing and documentation is complete for chronic care management eligible patients.
The position requires climbing, stooping, kneeling, crouching, reaching, standing, lifting, grasping, feeling, talking, hearing, repetitive motions, and finger use. Pushing and pulling are occasionally required. Use of a computer, keyboard, and telephone along with various office machines is an essential part of the job.
