The LPN provides comprehensive health oversight and hands-on care for participants in a residential mental health setting. Responsibilities include managing medication records, performing clinical nursing care, and collaborating with interdisciplinary teams to ensure resident well-being.
Candidates must hold a current Pennsylvania LPN license and CPR/First Aid certification. Additional requirements include a valid driver's license, FBI and Act 33/34 clearances, and completion of health screenings.
RHD PIER (Pledge for Independent Empowerment & Recovery) provides a full range of mental and emotional health services in a residential setting with ten beds. Participants enjoy individualized engagement to help them develop and achieve goals to allow them to increase their independence. RHD PIER is a contracted program through Bucks County Mental Health/Developmental Programs (MH/DP) and overseen by Pennsylvania policies for Long Term Structured Residents (LTSR). The PIER program is currently the only LTSR facility exclusively serving Bucks County, PA, residents.
The primary role of a Licensed Practical Nurse (LPN) is to provide comprehensive oversight, coordination, and hands-on care in addressing all health-related matters affecting program participants. This includes both direct and indirect health concerns, ensuring the physical and emotional well-being of individuals through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers. The LPN plays a critical role in maintaining a safe, responsive, and supportive environment that promotes optimal health outcomes for all individuals involved in the program.
Major Responsibilities: Maintains nursing documentation and medication records
Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded in electronic MAR
Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etc
Assesses residents’ health and emotional needs
Communicates with other members of the nursing and program staff; communicates serious issues to Nurse Manager and Program Director
Maintains confidentiality, including keeping voice low and not talking about residents in public areas
Other duties as assigned
