Description Schedule

• Full-time, 40 hours per week | Monday - Friday | 8am - 4:30pm
• Department: PACE of Southwest Michigan | Niles, MI

What You’ll Do

As a Registered Dietitian at PACE of Southwest Michigan, you'll help older adults maintain their health, independence, and quality of life through personalized nutrition care. Working alongside medical, nursing, rehabilitation, and social services professionals, you'll develop individualized care plans and play a vital role in supporting participants' overall well-being.

• Key Responsibilities Conduct comprehensive nutrition assessments upon enrollment, semi and annually, and with any significant change in participant condition.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide medical nutrition therapy (MNT) for chronic diseases.
• Monitor weight trends, laboratory values, oral intake, dentition, and nutritional risk indicators.
• Recommend therapeutic diets, oral nutrition supplements, and texture‑modified diets.
• Actively participate in IDT meetings, care planning, and case conferences.
• Provide individualized nutrition education and counseling to participants and caregivers.
• Conduct home visits as needed to assess food access, safety, and ability to prepare meals.
• Coordinate with home care teams to address nutrition‑related barriers in the community setting.
• Organize home delivered meals and supplements as required.
• Oversee the PACE kitchen area.
• Assist in monitoring meals including intake, diet compliance, and tray accuracy.

• Skills, Knowledge & Expertise Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field.
