Clinical Care Coordinator, CORE (ACSW/AMFT Required)

• The Clinical Care Coordinator is responsible for developing and providing high-quality therapeutic interventions and services in support of an individual’s wellness and recovery.
• Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support.
• Schedules may include after hours as needed by the program. ​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​Community Outreach Recovery Empowerment (CORE) is an adult outpatient program that provides moderate to high intensity mental health services to individuals 18 years or older who are eligible for Specialty Mental Health Services in Sacramento County.
• CORE services are focused on an individuals strength and recovery and provides flexible services to meet clients where they are at.
• CORE programs provide trauma-informed, culturally and linguistically responsive, recovery-oriented, community-based services, using community defined and best practices to all clients.
• Our 3 CORE sites have a co-located, peer run, Community Wellness Center (CWC), which acts as a drop-in center for all interested community members ages 18 years and older.
• At a Community Wellness Center, an individual can request assistance with housing, employment, behavioral health, symptom management, food, health, wellness and many other meaningful services.
• Each center includes peer-led activities, groups and experiences to promote Wellness, Recovery and Resiliency.

Monday - Friday Day Shift.

Duties and Responsibilities

Delivery of individual, family, group counseling, individual rehabilitation, and case management/brokerage for individuals within the Care Coordinator's caseload as assigned. Utilize Evidence-Based Practice for the designated population/program to assure high-quality results and intended impact. Assess an individual’s risk status including risk-based symptoms, substance use, environment, physical conditions, etc, and intervene appropriately. Creating a service plan that aims at an individual getting better/healthy and accomplishing service goals that are person-centered and belong to the client. Outreach and progressive engagement and reporting disengagement to the team immediately. Responsible for working with the outreach team and determining when to disenroll individuals based on program protocols. Assessment, screening referrals, service planning, community planning, and benefits coordination support wellness and recovery principles. Documents and maintains all data collection, reporting, and charting records in accordance with BACS and other relevant county regulatory policies/agencies. Participates in supervision, staff meetings, consultation, travel, crisis/emergency coverage, weekend coverage, as well as, agency-wide meetings and training, as required by the manager. Other duties as assigned.
