Provide comprehensive case management and psychosocial support for clients across the HIV continuum of care. Coordinate with community agencies and medical providers to reduce barriers to care and improve patient adherence.
Requires a Bachelor's degree in Social Work, Psychology, or a related field. At least one year of related human service experience is preferred, and bilingual proficiency in Spanish and English is sought.
Perform psychosocial intake assessments and develop care plans. Assist patients in improving their adherence to HIV care and other medical illnesses. Assess and refer patients for mental health and substance abuse programs. Assist patients in applying for health insurance and prescription programs such as ACA, MADAP, Medicare, Medical Assistance, Social Security Extra Help, QMB, SPDAP. Refer patients to the Department of Social Services for programs such as Food Stamps, TDAP, TCA, Eviction Prevention, and Energy Assistance. Provide education on the disability programs provided by the Social Security Administration, such as SSI and SSDI. Refer patients to community resources programs for housing/shelters, food services, and employment. Submit Ryan White voucher requests for rent/utility assistance and DME. Assess patients for medication purchase and co-payment vouchers. Arrange patient's transportation services that include cabs, tokens, and referrals to the Moveable Feast transportation program. Assist patients in enrolling in transportation programs offered by the MTA and Medical Assistance. Perform other duties as assigned or required.
Bachelor's Degree in Social Work, psychology, or related field. One year of related human service experience, preferably in case management. Additional education may substitute for required experience, and additional related experience may substitute for required education beyond a high school diploma/graduation. equivalent, to the extent permitted by the JHU equivalency formula.
