The RN Intake Resource Nurse manages new patient admissions, medication reconciliation, and care plan entry to ensure seamless transitions into home health care. They coordinate communication between physicians and the interdisciplinary team while providing triage support and clinical supply training.
Candidates must hold a current Registered Nurse (RN) license in good standing in New Hampshire. A minimum of 2 years of experience in home health care or a clinical setting is preferred.
The RN Intake Resource Nurse serves as a key point of contact for new patient admissions and ensures a seamless transition for individuals receiving home health care services. This role supports clinical operations by managing workflows and ensuring timely communication across the interdisciplinary team. The Resource Nurse is responsible for medication reconciliation, care plan entry, triage support, responding to supply-related clinical requests, and training new staff on clinical supply processes. This role promotes high-quality, compliant, and patient-centered care through strong clinical judgment and effective coordination. Key Responsibilities: Patient Intake and Assessment: Conduct initial referral assessments and review medical records to identify specific care needs. Collaborate with physicians, case managers, and other healthcare professionals to evaluate care requirements and establish patient care plans. 2. Coordination and Communication: Ensure compliance with company policies, state and federal regulations, and patient confidentiality requirements. Reviews all patient discharges to complete medication reconciliation, care plan updates, and order entry. Ensures all required documentation is completed accurately and submitted on time. Contact providers for Timely Initiation of Care (TIOC) orders and diagnosis changes when needed. Manages requests for multidisciplinary verbal orders, ensuring accuracy, and timely follow-through. Calls providers to obtain new discipline orders to support staff and patient care needs. Answers triage calls, gathers clinical information, and completes documentation promptly and accurately. Performs as needed nursing visits. 3. Quality Control and Documentation: Manages incoming orders by maintaining and processing order entry, including orders received via fax and phone. Replies to requisitions for additional clinical information related to patient supplies, ensuring timely and accurate responses. 4. Collaboration with Multidisciplinary Team: Work closely with case managers, registered nurses (RNs), therapists, and physicians to ensure the initiation of patient care plans. Participate in team meetings to discuss patient care and progress, and adjust plans as needed which includes nurse staff meetings and weekly IDG. Supports flu clinic operations alongside the Home Care Manager, including preparation, coordination, and clinical participation.
Licensure: Current Registered Nurse (RN) license in good standing in NH.
Minimum of 2 years of experience in home health care or a clinical setting preferred.
Excellent communication and interpersonal skills. Strong organizational skills and attention to detail. Ability to work independently and as part of a team. Proficiency with electronic health records (EHR) and other healthcare management software. Knowledge of home health care regulations and compliance standards.
