Summary: The Hagerstown Health Hub is taking an innovative approach to create rapid access to primary and mental health services in Washington County, Maryland, by co-locating these services with case management, housing, food, and workforce development services. The Health Hub Care coordinator will be responsible for patient intake, record keeping, and coordination of patient care with external stakeholders.
Essential
Functions: Complete intake documents with patients. Enter patient intake and appointment follow-up documents into appropriate systems. Patient appointment scheduling, including managing daily walk-in appointment schedule. Support the Health Hub Nurse Practitioner and LCSW in record keeping. Schedule patient appointments for secondary screenings. Coordinate patient care with external stakeholders, including scheduling patient transportation. Coordinate appointments with Health Care Navigator for uninsured patients. Participate in interdisciplinary team meetings with internal HGI stakeholders. Perform other related duties as assigned. Complete intake documents with patients Enter patient intake documents, appointment follow-up documents, and relevant patient information into appropriate systems Managing patient appointments, including daily “walk-in” appointment schedule and secondary screenings Support the Health Hub medical professionals as directed Coordinate patient care with external stakeholders, including scheduling patient transportation Refer patients to health insurance navigators as needed Participate in interdisciplinary team meetings with internal HGI stakeholders Maintain Health Hub office and supplies Attend Outreach events as directed Manage all walk-in’s to the Resource Center
