The Care Coordinator provides direct clinical support including vital signs collection and specimen preparation while managing care coordination for clients with behavioral health and chronic medical conditions. They facilitate referrals, track care gaps, and collaborate with multidisciplinary teams to reduce hospital readmissions and improve health outcomes.
Candidates must have a high school diploma or GED, Medical Assistant certification or relevant experience, and at least one year of experience in a healthcare or community health setting. Proficiency in EHR systems and basic medical terminology is required, with an associate degree and bilingual skills preferred.
Associate degree in Medical Assisting or related healthcare field. Experience in a Certified Community Behavioral Health Clinic (CCBHC), Federally Qualified Health Center (FQHC), community mental health center, or integrated care setting. Knowledge of behavioral health, substance use disorders, chronic disease management, and social determinants of health. Bilingual English/Spanish preferred. Experience with care management, population health, or case management activities.
