Position Overview As a Home Care Nurse, you will conduct client assessments, develop care plans, and collaborate with caregivers, clients, and families to ensure the highest quality of care.

Responsibilities

Conduct initial and ongoing client assessments Develop, implement, and update individualized care plans Perform annual evals and skill assessments for Caregivers Monitor client health status and document care accurately Communicate effectively with physicians, families, caregivers, and office staff Provide education and support to clients and their families Ensure compliance with state regulations and company policies

Qualifications

Active Massachusetts RN License Minimum of one year nursing experience preferred Home care, hospice, case management, or community health experience is a plus Excellent assessment, communication, and organizational skills Reliable transportation and valid driver's license Ability to work independently while maintaining strong teamwork
