The Primary Service Coordinator manages a caseload of youth and families, facilitating wraparound meetings and developing Individual Service Plans. They provide on-call crisis support and coordinate care across multiple service systems to keep children safely at home.
A bachelor's degree in a social services discipline is required, while a master's degree and clinical experience are preferred. Candidates must be eligible for Mississippi's DMH credentialing and possess a valid driver's license and automobile.
The Primary Service Coordinator: Carries a caseload of 10 to 12 youth/families Provides care coordination services by facilitating wraparound meetings Collaborates with the family and key players in the development of the Individual Service Plan (ISP) Develops and updates the Individualized Crisis Management Plan (ICMP) as needed Completes care coordination activities as identified in the wraparound meetings Provides transportation to appointments and activities as needed Provides on-call crisis support to the youth and family during the week (schedules vary by location) Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development and quarterly booster trainings to enhance clinical skills Completes accurate and timely documentation in an electronic medical record system (EMR) Performs other duties as assigned
Schedule is flexible and non-traditional as it is based around the availability of youth and families served. Applicants must possess a current, valid driver’s license, an automobile for work purposes, and proof of auto insurance. Community-based staff will be reimbursed for applicable mileage as commutes can range upwards of 60 to 80 miles.
