Provide daily personal care, health monitoring, and skill development for clients in their homes. Responsibilities include assisting with activities of daily living, implementing individual service plans, and documenting care electronically.
Requires at least one year of paid caregiving experience and a valid driver's license with reliable transportation. Active certifications such as DSP (DoDD), CNA, or STNA are highly desired, along with current CPR and First Aid certification.

Night/Overnight shifts are rarely available. Part-Time and Full-Time availability desired (based on client needs and availability). Occasional client-specific flexibility is needed between school year vs summer/holiday schedules (for pediatric clients), and general family respite needs and/or medical appointment changes. Local travel throughout Lucas County and surrounding areas. Requirements / Non-Negotiables Minimum of 1 year paid in-home care or caregiving experience Actively-certified DSPs, STNAs and CNAs w/ +1 year experience are highly desired (or HHA/PCA with 2+ years paid experience) DSPs also require a valid DoDD DSP certificate within the past 12 months and familiarity with Ohio Board of DoDD rules. Successful completion of all HCN skills tests and competency evaluations. Willingness and timely compliance to maintain all ongoing Training & Certification Requirements Active CPR + FA certification (+ proof/documentation of both readily available) Medication Administration Certification is highly preferred 18yo + active Driver's License / State ID Reliable transportation to all scheduled visits, within full reach of stated travel radius. Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid). Consent + Ability to pass a State & Federal background check (including fingerprints) Willing and able to regularly perform all physical demands of home care (including lifting/carrying up to 30–50 lbs. occasionally). Experience with modern EMR/EHRs is a plus. Must adhere to Individual rights protections (Ohio Bill of Rights for individuals with DD), Confidentiality (HIPAA compliance), and Mandatory reporting laws (abuse, neglect, exploitation). Must maintain professionalism with individuals, families, and care teams. A successful Home Healthcare Aide provides personal care services under the direction of the Registered Nurse or Therapist in their region. You will be assigned to specific clients by the Registered Nurse or other appropriate professionals and perform services for clients as necessary to maintain personal comfort and safety in their home. Report to: RN Case Manager / Administrator / Director of Nursing FULL REQUIREMENTS + QUALIFICATIONS: Successful completion of a formal certification training program and/or a written skills test and competency evaluation. CHHA, CNA, and STNA accreditation desirable but not required. Meet the training and/or certification requirements of the state in which the home health aide services are being provided. This includes specifications of contracted agencies for which services are being provided. Minimum of 1 year paid in-home care or caregiving experience. Active CPR + FA certification and documentation of both. Be at least eighteen (18) years of age. Current driver's license, good driving record, and/or reliable transportation is required. Exceptional on-time and attendance record. Consent + Ability to pass a State & Federal background check (including fingerprints). Demonstrated ability to read, write, and follow a written Plan of Care. Good interpersonal skills and a passion for caring for those in need. Willing and able to regularly perform all physical demands of home care (including lifting/carrying up to 30–50 lbs. occasionally). Willingness and aptitude to perform light housekeeping in addition to patient care services as needed. ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY: Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist. This includes opening a container or pill caddy & handing it to the patient, but does not include administration of the medications. Knows and follows infection control policies, including proper hand washing techniques. Reports any observed or reported changes in the client's condition and/or needs to the Registered Nurse. Documents care provided and completes the forms required for the client's records. Completes the appropriate records to document cares given and pertinent observations. Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested. Treats patients and families with respect. Maintains confidentiality in all aspects of the job. Accepts and fulfills assignments with the agency. Exercise good judgment in accepting assignments. Participates in required in-service programs to meet compliance requirements. Identifies learning needs to agency supervisor.
