We’re hiring an Ambulatory Clinical Pharmacy Specialist who will play a hands-on role in elevating patient care at a nationally recognized Academic Medical Center.
In this dynamic, patient‑centered position, you’ll build meaningful relationships with patients and providers while delivering high‑quality medication management.
You’ll work on‑site in clinic, collaborating directly with care teams and practicing under a Collaborative Practice Agreement (CPA) to conduct in-person and virtual patient assessments, order and monitor evidence-based treatment plans, optimize therapy, and resolve medication-related problems.

Salary: An employee in this position who performs the job duties in Washington can expect a salary rate between $130,000 and $150,000 (Hourly
Equivalent: Approximately $62.50 - $72.12 per hour, based on a 40-hour workweek.) plus bonus pursuant to the terms of an any bonus plan if applicable. The actual salary will depend on experience, geographic location, and other factors permitted by law.

What
You’ll Do: Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards. Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination. Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate. Support quality‑improvement and REMS initiatives. Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.
