The Nurse Practitioner manages oncology patients across hospital and office settings, implementing physician care plans and coordinating between inpatient and outpatient care. Responsibilities include performing triage, conducting patient visits, and managing clinical projects such as palliative care and survivorship programs.
Candidates must hold an active state Nurse Practitioner license in good standing and possess BLS or CPR certification. Demonstrated knowledge of NP theory, medical equipment, and OSHA regulations is required.
Provides direction to staff in regards to patient care.
Supports the Patient-Centered Medical Home (PCMH) by working with patients, families, providers and other staff to promote timely access to needed care, provide daily continuity of care coordination, and coach patients, families and care givers to understand the patient care plan and self-care management responsibilities.
