Social Worker - Full-Time - Onsite Skilled Nursing Facility The Social Worker provides medical social services to residents in accordance with physician orders and under the direction and supervision of the community's Social Services Manager. This position assesses residents' social, financial and emotional needs; and makes appropriate referrals for resolution of problems and facilitates aspects of case management regarding resident transitions through the continuum of care in the community.
