Physical Therapist (PT) – Home Health (PRN / Hourly) 📍 Lincoln City, Oregon (Oregon Coast) 🏢 Signature Healthcare at Home Position Overview Signature Healthcare at Home is seeking a compassionate and skilled Physical Therapist (PT) to join our Home Health team on a PRN/hourly basis in beautiful Lincoln City, OR.
This role offers flexibility and the opportunity to make a meaningful impact by providing patient-centered care in the comfort of patients’ homes along the scenic Oregon Coast.
The ideal candidate is independent, patient-focused, and passionate about delivering high-quality rehabilitative care to diverse populations.
Key Responsibilities Conduct comprehensive physical therapy evaluations in the home setting Develop and implement individualized plans of care based on patient needs and physician orders Provide hands-on therapy to improve mobility, strength, balance, and functional independence Educate patients and caregivers on treatment plans, exercises, and safety strategies Accurately document visits, progress, and outcomes in the EMR system Collaborate with interdisciplinary team members including nurses, OT, SLP, and physicians Monitor patient progress and adjust treatment plans as needed Ensure compliance with all regulatory, ethical, and company standards Qualifications Graduate of an accredited Physical Therapy program Current Oregon Physical Therapist license (or ability to obtain) Minimum of 1 year of experience preferred (home health experience a plus) Strong clinical assessment and critical thinking skills Excellent communication and interpersonal skills Reliable transportation, valid driver’s license, and auto insurance Ability to work independently and manage a flexible PRN schedule Schedule & Compensation PRN / Per Diem / Hourly – flexible scheduling based on availability and patient needs Competitive hourly rate (DOE) Mileage reimbursement available Why Join Signature Healthcare at Home?
