Patient Safety & Observation Provide continuous observation of assigned patient; never leave patient unattended Maintain awareness of safety risks including falls, elopement, seizure, or suicide precautions Follow all safety protocols and emergency procedures Accompany patients at all times, including during transport, ambulation, and personal care Basic Patient Care Support Assist with daily care including bathing, oral hygiene, grooming, and skin care Support ambulation, positioning, and transfers using proper techniques Assist with toileting needs and incontinence care Ensure patient supplies and equipment (e.g., TED hose, compression devices) are properly maintained Respond to call lights and communicate patient needs promptly to the RN Nutrition Support Assist with tray setup and patient positioning for meals Help patients with feeding as needed Assist with menu selection and ensure dietary orders are followed Communicate intake and output information to the RN Support tray distribution and removal as directed Communication & Patient Experience Maintain patient confidentiality at all times Communicate respectfully with patients, families, and care team members Act as a patient advocate by providing comfort, support, and orientation to the environment Notify RN of patient concerns, behavioral changes, or family questions Documentation & Reporting Document patient care activities, safety checks, and observations on treatment flowsheets Provide thorough verbal handoff reports at shift change Communicate observed changes in patient condition to the RN promptly Unit Support & Operations Assist with maintaining a safe, clean, and organized patient environment Follow infection control protocols, including proper PPE use and disposal of hazardous materials Participate in required training, in-services, and ongoing competency development Physical Requirements & Working Conditions Ability to stand, walk, bend, lift, and reach throughout the shift Ability to lift and transfer patients (up to 50 lbs. as needed) Exposure to blood, body fluids, and potentially hazardous materials Ability to work in a fast-paced environment and respond to difficult situations Ability to transport items and equipment as needed Ability to recognize and respond to patient needs across various age groups Being Part of Something Bigger At Advocate Health, your work matters—to patients, teammates, and the communities we serve.
