The pharmacist is responsible for filling drug orders, monitoring patient therapies, and providing clinical services across multiple pharmacy locations. They must collaborate with healthcare providers to optimize medication outcomes and supervise supportive personnel.
Candidates must be a graduate of an accredited pharmacy school, licensed to practice in Louisiana, and have at least one year of experience as a practicing pharmacist. Proficiency in English and a strong knowledge of pharmacology and drug interactions are required.
Perform all pharmacist functions as detailed in Louisiana Board of Pharmacy Laws and Regulations. Travel between assigned pharmacy locations (Lake Charles, Crowley, Lafayette pharmacy) to provide operational and clinical coverage. Provide clinical pharmacy services, including medication therapy management (MTM), medication reconciliation, and patient counseling. Support quality improvement, patient safety, and medication-use initiatives. Collaborate with providers and other healthcare professionals to optimize medication therapy outcomes. Evaluate appropriateness of drug therapy. Verify accuracy, efficacy, and safety of presented prescriptions. Perform comparative reviews of medications prescribed relative to patient's chart. Perform prospective drug utilization reviews and notify practitioners of over or underutilization, therapeutic duplication, drug or disease contraindications, incorrect dose or duration of treatment, drug-allergy interactions, clinical misuse and abuse, etc. Provide patient education as advised by the Louisiana Board of Pharmacy. Provide more advanced patient education to patients with two or more risk factors for cardiovascular events. Be responsible for manufacturer drug recall communications and returns. Observe prescribing trends and assist in inventory management. Develop and maintain effective communication with center staff and administration. Supervise and be responsible for supportive personnel. Remain current with developments and trends in the practice of pharmacy. Maintain knowledge of evidence-based clinical practice guidelines as published by medical experts. Be receptive and sensitive to patients with socioeconomic challenges. Assist Director of Pharmacy in other pharmacies. 20. Assist with patient related projects as assigned by the Director of Pharmacy.
