Youth and Family Skills Coach

Description Nexus Family Healing is looking to hire a Youth and Family Skills Coach to join our newest youth crisis and stabilization center in Hennepin County!

This new youth crisis and stabilization center is a 13-bed, short-term residential service is for Hennepin County youth with complex mental and behavioral health issues looking for appropriate care. The service will help to reduce the number of youth who are stuck in inappropriate care settings such as emergency rooms, police stations, juvenile detention or unstable home situations and provide treatment and therapeutic supports while triage, assessment, and transition planning take place with families.

• Primary responsibilities: Client Life Skills Support and Coaching Assists clients in using age-appropriate self-management skills, including communication, self-regulation, self-help skills, play, social, and relationship skills Provides de-escalation and therapeutic supports as needed with clients Notifies and collaborates with Clinical Supervisor in case of crises and emergencies Completes clinical documentation in compliance with program expectations; demonstrates strong written and oral communication skills to generate and support program documentation expectations and standards Program Facilitation & Consultation Attends and participates in clinical supervision and team case consultation meetings in accordance with program expectations Completes education and training experiences in accordance with program expectations Ensures minimum hours of direct services as assigned Maintains compliance with all agency, state, and regulatory standards and remain compliant with all HIPAA regulations Assists in implementing responses to youth behavior and other skills deficits according to the developed treatment plan
