RN Case Manager

• REGISTERED NURSE CASE MANAGER Are you passionate about helping those who struggle to access care or feel unwelcome in other healthcare spaces?
• Western North Carolina Community Health Services (WNCCHS) is a Federally Qualified Health Center (FQHC) serving underserved communities in Western North Carolina.
• WNCCHS is seeking a mission-minded Registered Nurse Case Manager to provide care management to patients with Medicare and Medicaid at our Minnie Jones Health Center in Asheville.
• RN Case Managers collaborate with medical, behavioral health, pharmacy, dental, and care management teams to provide customized care plans to improve health outcomes.
• The RN Case Manager ensures the successful transition of care from inpatient or the emergency department to culturally competent, comprehensive primary care.

Qualifications

• NC state RN licensure
• BLS certification
• Bachelor’s degree in nursing or related field
• Two years’ RN experience required
• Experience with Medicaid patients
• Experience in community health or outpatient care preferred
• Clinical experience in nursing care of the medically complex patient highly desirable
• Familiarity with role of FQHCs in the community
• Bilingual in English and Spanish and/or Russian is a plus
• HIV care experience a plus COMPENSATION
• Base Salary: $75,000 to $80,000 (based on 40 hours/week) WORK/LIFE/FINANCIAL BENEFITS
