  • Where You’ll Work Founded in 1887, Dignity Health - California Hospital Medical Center is a 318-bed, acute care, nonprofit hospital located in downtown Los Angeles, California.

  • PLANNING: Completes an individualized plan of care based on assessment of the patient.
  • Coordinates with other disciplines when indicated.
  • Prioritizes care needs and sets measurable goals and time frames.
  • Includes patient and family in the planning.
  • IMPLEMENTATION: Implements interventions based on nursing and medical plan of care.
  • Manages patient population, diagnosis and age specific needs through appropriate adaptation of interventions.
  • Appropriately utilizes equipment and supplies (including following established policies, procedures and protocols governing the transport, handling and storage of intravenous fluids).
  • EVALUATION: After implementation, reassesses the patient to verify effectiveness of interventions.
  • Updates the plan of care to reflect current care needs.
  • Demonstrates the ability to recognize and adapt to changes in patients condition (i.e..
  • LOC change, vital signs, test results)in a timely and clinically appropriate manner.
  • Uses SBAR to notify physicians and other providers of clinical findings.
  • Follow policies and procedures for contacting physician regarding critical values.
  • Uses appropriate resources and follows chain of command.
  • Recognizes limits of scope of practice.
  • Seeks multidisciplinary collaboration.
  • Integrates & interprets multiple sources of data to make appropriate clinical decisions based on patient needs & desired outcomes.
