Responsible for the safe and rational distribution of pharmaceutical agents to patients of all ages. Develops care plans using disease management guidelines and provides clinical support to physicians and nursing staff.
Requires a degree from an ACPE accredited pharmacy school and a valid pharmacist license from the Virginia Department of Health Professions. At least two years of experience in a hospital clinic or pharmaceutical setting is required.
Position Title: Clinical Pharmacist (Per-Diem)
Purpose & Scope: Responsible for the safe, accurate and rational distribution of pharmaceutical agents to patients of all ages. Develop and implement a plan of care utilizing disease management guidelines and standards. Provide pharmaceutical care and advanced clinical support to the nursing staff, physicians, and patients. This shall be carried out in accordance with all state and federal laws, JCAHO standards, and hospital / departmental policies and procedures.
Graduate School (College) Of Pharmacy Accredited by ACPE is required. Doctorate in Pharmacy or Pharmacy Residency is preferred.
