Assess and counsel medically complex patients to develop individualized treatment plans for disability adjustment and behavior modification. Coordinate resource linkage and work within interdisciplinary teams to provide care in home and community settings.
Candidates must hold a Master's degree in Social Work and possess a valid state licensure as an LCSW. Strong skills in planning, organization, writing, and professional communication are required.

We are looking for experienced Licensed Clinical Social Workers who want to provide truly patient-centered care in Vista, CA and surrounding areas- with PRN positions available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
