The RN evaluates the medical and physical needs of consumers in mental health and substance abuse treatment, administering medications and injections. They coordinate care with external health providers and collaborate with a multidisciplinary team to develop comprehensive treatment plans.
Requires a Bachelor's degree in nursing, active RN licensure, and at least one year of experience with mental health or substance use populations. Proficiency in electronic medical records and strong communication skills are essential.
The RN will be a clinical staff member who, in addition to performing routine team duties, has responsibility for evaluating the general medical and physical needs of assigned consumers for treatment; and renders general nursing care (e.g., administers injections and medications) on an on-going basis. Provide medical assessment and treatment of adults engaged in mental health and substance abuse treatment, under the supervision of NP/Psychiatrist. Essential Accountabilities: Assist the prescriber on the ARS team with assessment of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinating medical treatments with psychiatric treatment under the direction of psychiatrist/NP. Is primary staff to coordinate and collaborate with FQHC, private practitioners and other health providers to share information/data, with consent, work in partnership for wellness and insure follow up for medical concerns. Complete nursing assessment within 30 days of admission and as required by regulatory agency. Collect all relevant health data and measurements in compliance with grant/state funding. Collaborate with the treatment team in developing a comprehensive assessment of psychiatric history, mental status, and diagnosis; physical and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living and family structure and relationships. Perform individual, group and family supportive therapy and illness education in the process of carrying out the tasks above. Transcribes the prescriber’s medication orders if indicated; administers prescribed injections and medications as needed. Observes consumer, records significant conditions and reactions, and notifies current prescriber of consumer’s condition and reaction to drugs, treatments, and significant incidents. Monitor and collects specimens for clients as needed. Instructs consumers about daily medications (e.g., names of drugs, dosages, interactions, benefits). Reviews medication record of each consumer for potential adverse reactions, allergies, interactions, and contraindications monthly under the direction of the psychiatrist/NP. Communicates with prescriber regarding any medication discrepancies. Records vital statistics and other pertinent data on consumers; maintains records of same. Takes temperature, blood pressure, pulse, weight, and other vital signs to detect deviations from normal and determine progress of consumers. Assists prescriber in evaluation for MAT services including collecting pertinent medical information, referring to outside medical providers and performs necessary vitals as requested. Liaisons with physician, dietitians, hospitals, pharmacies, and the consumer’s family on all health situations. Document any progress notes in EHR within 48 hours of all client and collateral contacts Provides networking and linkages between internal and external programs. Attend and participate in all team meetings. Acts as a role model for participants; furthers participants in researching and obtaining community resources appropriate and applicable to their needs and goals; follows-up on any aftercare/recommendations for treatment. Provides crisis intervention and mental health counseling as needed. Knowledge of evidenced based treatment practices and willingness to develop skills. Collaborates with treatment team to insure approach is consistent. Staff person may take part in case conference meetings and supervision with program director and psychiatrist/NP. Demonstrates knowledge of adult psychiatric disorders, substance abuse use and treatments. Is culturally competent and sensitive, practices a trauma informed approach that is individual, and family driven, youth guided, strength based and trauma informed. Adheres to confidentiality laws and guidelines. Will be trained in suicide prevention, intervention and post-vention and expected to be a team member in addressing clients and families as needed. #INDClinical Job Qualifications Bachelor’s degree in nursing, RN licensure, and at least one year of experience working with individuals with mental health and / or substance use involvement. Clinical supervisory experience is desirable. A clear understanding or demonstrates a willingness to learn about various program models to include but are not limited to ACT and PROS. Willingness to learn and demonstrate use of evidenced based treatment modalities. Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record.
