Perform in-home patient assessments and provide skilled nursing care, including wound and catheter care. Educate patients and caregivers while maintaining accurate documentation and updating individualized care plans.
Requires an active Texas RN license and a valid driver's license with reliable transportation. Experience in home health or acute care and proficiency with Kinnser/WellSky are preferred.
