Evaluate patients to develop and implement individualized in-home physical therapy plans to improve mobility and independence. Monitor patient progress and provide education to both patients and caregivers on safety and exercise techniques.
Candidates must hold an active Texas Physical Therapist license and possess a valid driver's license with reliable transportation. Experience in home health and proficiency with Kinnser/WellSky EMR are preferred.
