The Services Coordinator creates linkage systems with community agencies to help clients access basic needs, housing, and medical services. They are responsible for managing authorizations, coordinating discharge plans, and facilitating psycho-educational groups.
Candidates should have a Bachelor's degree in a related field and 2-3 years of case management experience in substance abuse or mental health. Proficiency in navigating the CT system of care for Medicaid members and knowledge of MAT and WRAP is preferred.
Liberation Programs is one of Fairfield County's leading behavioral health organizations specializing in treatment for all types of substance use disorders including alcohol, opiates, depressants and stimulants. Our goal is to help individuals and families overcome addiction in order to restore their lives and ultimately strengthen our communities. Liberation provides services for youth, adults and families that include two inpatient treatment programs, outpatient and intensive outpatient services, health education for older adults and people living with HIV/AIDS and other chronic illnesses, treatment and resources for adolescents and their families, education and prevention efforts in the community, and permanent supportive housing for families. In operation since 1971, the agency has sites in Greenwich, Stamford, Norwalk and Bridgeport and served more than 2,400 individuals in our last fiscal year.

Under the direction of the Program Director, creates and maintains an appropriate linkage system with various community agencies to assist clients in obtaining entitlements and other basic needs including housing, job placement and vocational training, medical services, transportation, education, money management, and life skills. The Services Coordinator supports and enhances the client’s engagement in treatment and also plays an essential role in case management.
Duties: and Responsibilities:
1. Complete and file Authorization Forms for prospective and current residential clients 2. Coordinate care with counselors and discharge planner to ensure that authorizations are documented correctly 3. Review all initial and con-current authorization forms for accuracy and completeness.
