Provide skilled nursing care and comprehensive assessments to patients in their homes to support recovery and chronic disease management. Collaborate with physicians and interdisciplinary teams to implement individualized care plans and supervise home health aides.
Requires a current RN license, valid driver's license with auto insurance, and CPR certification. A minimum of 2 years of home health experience and familiarity with HomeCare HomeBase are preferred.
As a Home Health RN, you will provide skilled nursing care, complete comprehensive assessments, coordinate individualized care plans, and collaborate closely with physicians, caregivers, and interdisciplinary teams to support patients through recovery, chronic disease management, and overall wellness. The Company recognizes the use of personal vehicles by personnel for business purposes as part of their employment. The company will reimburse the eligible employee $0.50 per
mile to cover personal care use, auto insurance, depreciation, and gas. Each employee who is eligible for mileage reimbursement from the Company will be required to track and report mileage
accurately and timely each pay period. All mileage shall be calculated as follows:
