Provide after-hours skilled nursing care to hospice patients in various settings to support comfort and dignity. Collaborate with an interdisciplinary team to implement individualized care plans and educate caregivers on symptom management.
Must hold a current Ohio LPN or RN license, CPR certification, and a valid driver's license with insurance. A minimum of one year of nursing experience is preferred along with proficiency in electronic documentation.
Ohio’s Hospice is seeking a compassionate and dedicated LPN or RN to provide after-hours nursing support for patients receiving hospice care in the New Philadelphia and Wooster communities. This driving position offers an opportunity to make a meaningful difference in the lives of patients and families by delivering exceptional care during some of their most important moments.
The ideal candidate is committed to hospice philosophy, values patient-centered care, and is comfortable working independently while collaborating with an interdisciplinary care team.
