The Nurse Navigator serves as the first clinical contact for hospice patients, conducting initial assessments and developing plans of care. They coordinate admissions with external medical professionals and provide essential education and support to families and caregivers.
Candidates must be a graduate of an accredited nursing program with a current North Carolina RN license. At least 2 years of nursing experience is required, with a preference for those with hospice or home care backgrounds.
