The LPN delivers age-appropriate care to patients in home settings by implementing plans of care established by Registered Nurses. Responsibilities include documenting treatments, educating patients and caregivers, and communicating progress to the interdisciplinary healthcare team.
Candidates must be a graduate of an accredited nursing school with a current New Hampshire or compact state nursing license. A valid driver's license, reliable transportation, and one year of recent medical/surgical or acute care nursing experience are required.

Reporting to the Home Care Director/Manager, the Granite VNA Licensed Practical Nurse (LPN) delivers age appropriate care to the patient in the home setting utilizing standard nursing best practice: implementation of plan of care as established by a Registered Nurse, and within scope of practice defined by the NH Board of Nursing. LPNs will effectively interact with patients, care givers and other healthcare team members while maintaining all standards of professional nursing. LPNs are supervised by the Registered Nurse assigned to the patient.
Graduate of an accredited school of Nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC). One year of recent general medical/surgical or skilled/acute care nursing experience. Valid driver's license, reliable transportation and an acceptable driving record.
