The Resource Specialist assists patients and multidisciplinary teams in identifying and accessing tangible and supportive healthcare services. Key duties include conducting needs assessments, coordinating community resources, and supporting discharge planning and care transitions.
A Bachelor's degree in Social Work, Human Services, or an equivalent combination of education and experience is required. Candidates must possess excellent interpersonal, communication, and organizational skills.
