The Nurse Liaison builds personal relationships with elderly patients to promote comfort, safety, and dignity. They manage the admission process from acute care to long-term care and conduct resident assessments.
Candidates must be a licensed RN or LPN with at least one year of experience in patient assessment. The role requires the ability to travel 75% of the time and possess strong communication skills.

Nurse Liaisons must be licensed in their practicing state as an R.N. or LPN with a minimum of one year experience in resident/patient assessment. Nurse Liaisons will maintain an appropriate level of knowledge about sources of payment and must understand the admission process from acute care to long-term-care. They will be able to travel 75% of the time on day trips to hospitals, etc. Nurse Liaisons have the ability to read and write well enough to follow written directions, do charting and resident assessments. Verbal communication and presentation skills are important to carry on conversations with hospital patients, residents, co-workers and family members in an understandable manner. Nurse Liaisons are continually learning and growing by participating in educational sessions, in-service training and demonstrations.
