As a Hospice CNA, you'll work closely with patients in their homes and care facilities, assisting with personal care needs while providing comfort, dignity, and companionship. • Primary schedule: Part-Time/As Needed/Flexible to your schedule
• Territory: Clare/Mt. Pleasant Key Responsibilities:
• Provide personal care services including bathing, grooming, dressing, toileting, transfers, and mobility assistance.
• Support patients with activities of daily living while promoting comfort, dignity, and independence.
• Observe and report changes in patient condition to the clinical team in a timely manner.
• Provide compassionate emotional support to patients and families throughout the hospice journey.
• Maintain accurate and timely documentation while adhering to hospice policies, HIPAA requirements, and safety standards.

Qualifications

• Active CNA certification required.
• High School Diploma or GED required.
• Graduate of an approved nursing assistant training program.
• Hospice, home health, long-term care, or direct patient care experience preferred.
• Valid driver's license, reliable transportation, and current automobile insurance required.
