Manages a caseload of post-release services for children and sponsors, conducting interviews and home visits to ensure safety and well-being. Provides clinical case management, crisis intervention, and coordinates with external agencies and resource providers.
Requires a Master's in Social Work or a related behavioral science degree, or a Bachelor's with at least 5 years of clinical experience. Must be fluent in English and Spanish and have experience working with immigrant populations and trauma-informed care.
SALARY: $88,562 Annually
Work schedule: Five Days per Week, 40 Hours per Week
The PRS Case Manager (Level III) must have experience in trauma informed care, child welfare practices, managing cases, and conducting home studies according to the requirements in the state where services are being provided. Must be skilled at writing professional, thorough assessments reports. PRS Case Manager will have knowledge of local social services, community outreach and experience working with immigrant populations. Case Manager provides intensive case management services and is knowledgeable in family preservation, kinship care, and has experience in connecting and collaborating with local resource providers in providing post release services for children and their sponsor.
