Provide skilled physical therapy services and individualized treatment plans to help home-bound patients regain independence and mobility. Collaborate with an interdisciplinary team to monitor patient progress and reduce hospital readmissions through education and intervention.
Must possess a current Physical Therapist license in the state of employment, a valid driver's license, and automobile insurance. Candidates must be able to travel to patient homes and meet the physical demands of the position.

Develop, implement, and adjust individualized treatment plans based on patient needs and progress. Perform OASIS assessments and maintain accurate clinical documentation. Monitor, evaluate, and communicate patient progress, changes, and responses to treatment. Patient Outcomes Help patients reduce pain, restore function, improve mobility, and maximize independence through evidence-based therapeutic interventions. Utilize treatment techniques including therapeutic exercise, manual therapy, and other physical modalities as appropriate. Patient Safety & Risk Reduction Support efforts to reduce hospital readmissions through proactive patient education and early intervention strategies. Promote safe practices and preventative health measures. Patient & Family Education Educate patients and caregivers on disease management, treatment plans, home exercise programs, and the proper use of assistive devices including walkers, wheelchairs, canes, braces, crutches, and orthotic/prosthetic equipment. Collaboration & Supervision Collaborate with physicians, nurses, physical therapist assistants (PTAs), and other healthcare professionals to ensure coordinated patient care. Provide supervision and oversight of therapy services delivered by PTAs and other support staff as appropriate. Compliance & Professional Development Complete all required clinical documentation in accordance with agency policies, Medicare guidelines, and regulatory requirements. Maintain compliance with all applicable laws, regulations, and organizational standards. Participate in required training, meetings, and continuing education activities. Qualifications Required Current Physical Therapist (PT) license in the state of employment. Valid driver's license and current automobile insurance. Ability to travel to patient homes within assigned service area. Ability to meet the physical demands of the position. Preferred Skills Excellent communication and interpersonal skills.
