The Mental Health Professional develops individualized treatment plans and provides therapy and medication management for individuals with severe mental illness. They collaborate with a multidisciplinary team to deliver comprehensive services within home and community settings.
Candidates must hold a Master's degree in counseling, social work, psychology, or a related field and have 2 years of clinical experience with adults. A valid driver's license is required for travel across Carbon, Monroe, and Pike counties.

Our Assertive Community Treatment (ACT) model supports individuals with serious mental illnesses who may not have experienced success or haven't found effective support within traditional outpatient programs. We offer tailored services emphasizing relationship-building and active involvement in enhancing functioning and symptom management.
Services involve weekly home visits, provided either in individuals' residences or within the community, utilizing personal vehicles for client transportation and community-based tasks.
