Provide high-quality primary and palliative care to patients with chronic conditions through home visits. Responsibilities include performing physical exams, developing treatment plans, and coordinating with clinical operations.
Requires a Board Certified NP or PA certification and 1-2 years of primary care experience with geriatric patients. Strong clinical skills and the ability to operate independently are essential.
