Conduct comprehensive assessments and develop personalized care plans to improve member health outcomes. Coordinate care across providers and community resources while removing barriers to wellness.
Requires an active Virginia RN license, an Associate Degree in Nursing, and at least 3 years of clinical nursing experience. Candidates must reside in the Annandale/Springfield area and be willing to travel up to 75% locally.
Experience in case management, particularly within an integrated care model Bilingual (English/Spanish) Experience working with youth with mental health diagnoses Ability to thrive in a fast-paced, dynamic environment and manage multiple priorities effectively Education: Associate Degree in Nursing (required) Bachelor of Science in Nursing (BSN) (preferred) Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
