The role involves providing professional social services such as psychosocial assessment, patient advocacy, discharge planning, and crisis intervention to meet the varying psychosocial needs of patients and their families transitioning from acute hospital settings. Additionally, the worker will deliver professional social services to high-risk outpatients across several specialized centers.
Candidates must possess a Bachelor's Degree in a relevant field like Social Work, Sociology, or Psychology, with 3-5 years of clinical experience being preferred. Essential requirements include basic computer skills, effective communication abilities, thorough assessment skills, demonstrated leadership, and excellent time-management capabilities.
Bachelor's Degree in Family and Consumer Science, Social Work, Sociology, Psychology, or a related field. 3-5 years of clinical experience preferred. Basic computer skills.
