$55,000 - $65,000 / YEAR
Provide home-based care to patients with illnesses, injuries, and chronic diseases under the supervision of an RN, which includes taking vitals, performing assessments, documenting progress, and assisting the RN with the care plan. Responsibilities also involve educating the family on the care plan, medication, and home safety, while collaborating with the interdisciplinary team for care coordination and discharge planning.
Candidates must possess an active, unrestricted Licensed Practical Nurse (LPN) license in the State of Michigan and have at least one year of clinical experience as an LPN. Applicants are also required to hold current CPR Certification and maintain reliable transportation with a valid driver's license and current car insurance.
