The Hospice APP will evaluate, interpret, and coordinate information to make decisions regarding patient care. Responsibilities include shared call amongst providers and a mix of inpatient and outpatient care.
Candidates must be a Nurse Practitioner or Physician Assistant, board eligible or certified, and hold an unrestricted medical license in Iowa. Experience with palliative care and medicine management is preferred but not required.
Shared call amongst the providers, Triaged through IntelliTriage Practice is a mix of inpatient and outpatient, hospital, SNFs, home visits, assisted living facilities, etc. Ability to work as a team member to improve the health of the people and communities we serve Experience with palliative care, medicine management preferred but not required Evaluates, interprets and coordinates information to make decisions and recommendations regarding patient care Patient-focused, friendly and willing to collaborate in a team environment Excellent communication (written and verbal), analytical and problem-solving skills
